When is tinnitus a warning sign that needs an ENT doctor?
See an ENT specialist first if tinnitus is in one ear only, pulses with your heartbeat, started with a sudden hearing drop, or comes with dizziness, ear pain, discharge or facial weakness.
Red flags: tinnitus in one ear only or with one-sided hearing loss (may need an MRI to rule out a nerve tumour such as acoustic neuroma); pulsatile tinnitus that beats with your heart (may need vascular assessment); tinnitus with sudden hearing loss (see an ENT the same or next day); tinnitus with vertigo, ear pain, discharge, or facial weakness; tinnitus after a head injury.

For tinnitus linked to inner-ear hearing loss, the evidence-based approaches are sound enrichment, hearing aids when hearing loss is present, education and counselling, and cognitive behavioural therapy (CBT), which has the strongest research support for reducing distress.

Avoid silence, protect your ears from loud noise, keep a regular sleep routine and reduce stress. Small daily habits make a real difference to how much tinnitus bothers you.
Avoid complete silence, especially at night. A fan, soft radio or a sound app at low volume helps the brain tune out the ringing. During load-shedding, a battery-powered speaker or phone can keep gentle background sound going.
Protect your hearing around generators, workshops, loud weddings and firing. Foam or custom earplugs are inexpensive insurance.
Keep personal audio at a moderate volume and take listening breaks.
Keep a steady sleep routine; tinnitus is more troublesome when you are tired.
